捐款方法 Donation Methods

1. 請用劃線支票、匯票或本票，抬頭請寫「崇基學院」，並於支票背後寫上「崇基神學教育基金」、姓名、聯絡電話及地址，寄回以下地址：

   香港新界沙田
   香港中文大學
   崇基學院神學院
   註：「崇基神學教育基金」

   You may make a crossed cheque payable to “The Trustees of Chung Chi College”. Please write down “Chung Chi Theological Education Fund”, your name, contact number and address on the back of the cheque. The cheque and payment voucher should be sent to:

   Divinity School of Chung Chi College,
   CUHK, Shatin,
   NT, HK.

   Please state “Chung Chi Theological Education Fund” on the envelope.
2. 直接存入「崇基學院」銀行戶口，並取回存款收據正本，於正本背面寫上「崇基神學教育基金」、姓名、聯絡電話及地址，寄回以上地址及註明捐款目的。恆生銀行戶口號碼如下：

   戶口名稱：崇基學院
   戶口號碼：283-174001-001

   Upon making a deposit, please take the original bank transfer receipt. Please make a direct deposit to the Hang Seng Bank account at:

   Account Name: The Trustees of Chung Chi College
   Account No.: 283-174001-001

   Kindly write your name, contact number and address at the back of the receipt, and send it to the above address. Please state “Chung Chi Theological Education Fund” on the envelope.

索取捐款收據 To Obtain a Donation Receipt

捐款港幣一百元或以上可獲發收據作免稅用途。如欲索取捐款收據，請下載並列印此表格，連同支票/存款收據正本寄回本院。如就索取收據有任何查詢，請與本院職員聯絡，電話：3943 6706。

To obtain a donation receipt for tax deduction (for donations of HKD 100 or above), please send the crossed cheque or original bank transfer receipt together with this donation form to our school by post. For any enquiries, please contact us at 3943 6706.
